Role Description

The Case Officer promotes and restores patients’ health by developing day-to-day case management for non communicable diseases, Malaria and Covid 19 ;  Further he or she collaborates with physicians in a multidisciplinary manner by providing timely case indexing for Malaria and typhoid by ensuring that clients make informed  decisions when consenting.

Further the case officer takes an extra mile in interacting with patients for appointment bookings, monitoring of health conditions and do timely reporting and  escalation when need arises., He or she facilitates multi sectoral care for clients with chronic illnesses like cancer and also does the tracking for weekly reports and facilitates logistical and administrative necessities for clients needing review.

Key Role Accountabilities 

  • Always adhere to the MBHS Standards and standard operating procedures relating to your work.
  • Coordinate  and do home follow ups  through effective communication  on staff/ patients from home in an event of covid, post Cesarean or chronic illness.
  • Maintain confidentiality of any and all information relating to patient medical records and any MBHS business. Failure to comply will result in disciplinary action.
  • Work with the Human Resource Department to ensure clients and staff in quarantine are comfortable and are  well taken care of on a daily basis.

Key Responsibilities

  • Implementing daily allocations as assigned by the Unit leader in terms of MBHS protocols  and routine activity checklists .
  • Coordinating with patients and doctors for smooth flow of Malaria indexing and reporting.
  • Ensuring that data being Updated on a Malaria, Cancer and covid E- Registers  is of quality .
  • Ensuring all patients under chronic registers are given appointments for review and makes calls to remind clients for due dates.
  • Understanding and answering patient’s queries through professional phone calls.
  • Attending queries on emails and phone calls related to any project in the department.
  • Escalate challenges to Case Officer (Unit Leader) on Issues above the job role.
  • Ensuring Patient’s calling for results, indexing and appointments are attended to amicably.
  • Having an overall responsibility to work in the team  activities.
    Available to out of office programs for health promotions and Vaccination.

Qualification, Experience and Skills Requirements

  • Must have a full Grade 12 certificate completed with a credit or above in
  • English language and Mathematics.
  • Registered Nurse qualification
  • 1-2 Years Nursing experience
  • Good communication skills
  • Must be computer literate – Familiar with MS Suite
  • Experience in counseling preferred
  • Current NMCZ Licence
